33. A 27-year-old man with type 1 diabetes mellitus is brought to the emergency department because of stupor and diabetic ketoacidosis. His diabetes is usually well controlled. His wife says he has had a fever and productive cough for several days and has been taking an over-the-counter cough syrup. Which of the following is the most likely cause of the ketoacidosis?

I know the most common cause is failure to take insulin. But in this case, infection? I'm not sure, however.

In most inner-city practices, the most common cause of DKA is noncompliance with insulin therapy and is usually easily treated. stress from infection produces catecholamines, glucocorticoids. This in combination with insulin deficiency produces hyperglycemia and ketoacidosis

Any stress may lead to ketoacidosis in DM1 but infection is the most cause

yes... we get carried away by these stupid things and don't think logically. of course it's C

i don't think infection linked increase in stress hormones would take him to hyperketotic coma if he is compliant with insulin. they can increase glicemia, also lack of exercise in an active diabetic could lead to increase in blood glucose, but not as such high levels
