Computed tomography (CT) scan — A CT scan is a radiologic test that creates a three dimensional image of bodily structures. A particular type of CT scan (called noncontrast helical CT) without contrast can visualize almost all kidney stones, including those that may not be seen by other tests, as well as the presence of urinary tract obstruction. This test has become the gold standard for the initial radiologic diagnosis of stone disease; this test may not be necessary if a stone has been detected by one of the above tests. Moreover, all stone compositions, including uric acid stones, can be visualized with a CT scan.

Rule of 80-20: 80% of kidney stones are visible on x-ray and 20% on sonogram, For gall stones it is reverse 80% are visible on sonogram and 20% by x-ray. Why should CT-scan become the gold standard for the inditial radiologic diagnosis of stone disease?

thanks for the replies. yes, uric acid stones can be seen on the USG and the CT scan and i confirmed this after going thru some text. well the reason i asked this because i got confused with this question.

a 45 yr old man comes to the ER due to sudden onset of vomiting and severe left flank pain radiating to the groin. CT scan of the abdomen shows a stone. urinalysis shows- pH normal, esterase -ve, nitrites-ve, bacteria 1-2/hpf, wbc 1-2/hpf. which of the following stones is highly unlikely in this patient.

1-uric acid stone

2-calcium oxalate

3-struvite stone

4-cysteine stone.

i will not ask my confusion right now as it will give away the answer. so i'll let you all solve the question first.

CT scan finding only confirms presence of a ureteric stone(any type)...wch is the cause of his symptomatology. Thus giving us the diagnosis in this Q.
However ths Q actually is finding out from us, if we r aware of the various urinary characteristics wch form a suitable environment for the diffrnt types of stones. Like in this case, a normal pH wud r/o uric(needs an acidic pH) and r/o struvite(needs alkaline). Thn to r/o frm those to, u know struvite forms in the presence of an UTI, and the U/A here is clean...
maybe others can add more.

the only stone that can not be seen on CT is the stone from INDINAVIR (HIV DRUG) use

struvite stone is unlikely because the normal PH
