In thrombotic stroke of the brain the infarct remains pale. If the clot desolves(most of the time it doesn't) then it becomes hemorrhagic. i just don't get the concept of how a pale infarct becomes hemorrhagic after reperfusion. is it becaue the vessels is damaged and the blood leaks into the brain tissue? plz someone give a brief explaination. thanks One other question. in a pale infarct of the brain, heparin therapy is given. If you give heparin therapy, that should cause the thrombus to disovle, right? if yes, then that should produce hemorrhagic infarct after therapy, right? someone plz confirm this. thanks

u have the explanation right actually after throbolysis, when reperfusion occurs blood can enter the ischemic and hence the infarct would be hemorrhagic. a possible risk of thrombolysis is a hemorrhagic stroke. and prior to starting therapy for an ischemic stroke, a haem stroke has to be ruled out.

so i don't get it though. If someone has a pale infarct and u give them heparin, they r at risk for hemorrhagic infarct. thus, shouldn't the heparin therapy kill them?
