A 67 - year - old homeless man reports to the physician with a 1 - week history of a productive cough, shortness of breath and high fever. He denies alcohol, drug or tobacco use. The patient says that the onset of symptoms was sudden. A chest x - ray in the emergency department shows a large left lower lobe opacity. Which of the following antibiotics would best initially treat this patient's infection? A. Azithromycin B. Ceftriaxone C. Doxycycline D. Erythromycin E. Levofloxacin According to Kaplan Internal Medicine Notes, the drugs of first choice for empiric therapy of community - acquired pneumonia in an outpatient are macrolides, such as azithromycin and clarithromycin (new fluoroquinolones, such as levofloxacin, moxifloxacin and gatifloxacin, are alternatives). Why the correct answer is B, according to USMLE Rx, where I took this question from? The explanation says only that this is a case of typical pneumonia and common bacteria include Streptococcus pneumoniae, Staphylococcus aureus and Haemophilus influenzae, which could be covered with ceftriaxone. Shouldn't second or third generation cephalosporins be a good empiric therapy of community - acquired pneumonia in a hospitalized patient?
