yeah...star1 is correct I thought of dissecting aortic aneurysm as the pain was first radiating to the upper back and then it moved downwards and at those places the blood supply to the branches of the aorta will be compromised and I could not relate any of the other symptoms with such a presentation and also with bilateral flank pain and hematuria.
