Which vitamin defidiency would be in the differential for tabes dorsalis and cause loss of reflexes and loss of sensation?

Tabes dosalis? That's teritary syphilis......i am not sure if vitamin has to do with it. Are you trying to ask about Subacute Combined degeneration? That is related to V-B12 deficiency and it is usually associated with perniciou anemia.
1.) Demyelination of Dorsal columns => loss of vibration and position sense
2.) Demyelination of spinocerebellar tracts => arm & leg dystaxia
3.) Demyelination of corticospinal tracts => spastic paresis

Vitamin E- (I saw this in some Goljan notes) Vit. E deficiency is most commonly due to malabsorption. Clinical findings include
1)peripheral neuropathy
2)gait disturbance
3)loss of reflexes
4)loss of sensation
5)degeneration of posterior columns*
6)in premature infant may cause hemolytic anemia
7) in kids with Cystic fibrosi deficiency may produce brown bowel syndrome due to increase in lipofuscin
IN GENERAL ITS THE ANTIOXIDANT THAT NEUTRALIZES FREE RADICALS

Thats right. If you see demyelination associated with a vitamin deficiency, think E.

isnt the pathogenesis of B12 neuropathy also associated with demyelination?

B12 def. related neurological features are...
* decreased vibratory sensations( post. column)
* upper motor neuron signs(CST)
*dementia,impared memory, depression,psychosis.

I don't remember about demyelination :roll: ,will find it out

I dont think so, because it is easily reversible (i think).
