Dilated cardiomyopathy secondary due to MI is the most ominous sign of pulsus alternan.

Your answer of tamponade is correct but tamponade is hardly an ominous sign !

Ominous sing means you are at the DEAD BED and dying NOW !

You are going to die means ominous sign !

Clearly tamponade is not the ominous sign !
