IT ALWAYS COME WITH GENERALIZED LYMPHADENOPATHY? FOR EXAMPLE BURKITTS LYMPHOMA ,HAS GENERAL LYMPHADENOPATHY?
AND WHEN IT PRESENT IN GIT OR CNS OR BONE MARROW ,IT IS METASTATIC FORM OR PRIMARY SITE??

NonHodgkin lymphoma can be nodal or extranodal.
Extranodal - it means that it is primary there (outside of the lymphoid tissue), and lymph node isn't affected.
Why would NHL always go with general lymphadenopathy? We know for Staging Ann Arbor system where the first stage means one group of lymph nodes affected, stage two means two groups of lymph nodes affected in the same side of diaphragm...

but i didnt get second paragraph ,so if the staging system depend on how many lymph node involved ,so what stage will be the disease when it started extra nodal( no node involvment)?
previously i was understand it that it always (in all types) started with lymph node involvment and once it give extra nodal so it is metastatic!

STAGE I involvement of single lymph node region or single extallymphatic organ or tissue
II ......2 or more LN regions on the same side of diaphragm alone or with limited contiguous exteralymphatic involvement
III both sides of diaphragm
IV multiple or disseminated fociof one or more extralymphatic organ or tissue with or without lymphatic involvement
p.s i m just reading this off...the diaphragm is important btw ii and iii.
if u find an extranodal source i guess they'll have to telll u if its primary or metastaic and then we will classsify accordingly

the dif btw hodgkin's (HD) and NHL is based on histology. Reed Steenberg cells are present in HD
clinically however look for a few pointers.
HD usually has cervical or supraclavicular maybe mediastinal lymphdenopathy.
it frequently involves lymph nodes, with orderly anatomic spread.
NHL diffuse lymphadenopathy
nodal and extranodal disease
non-contiguous spread

but can u tell me the burkitt's lymphoma when appear in mandibule it is its primary site or first it be as lymphadenopathy then make metastasis there?

Primary extranodal lymphoma = when lymphoma arises in extrelimphatic localization (e.g.liver, cervix, GIT...) and can't be found in lymph nodes in 6 months.
Staging system is as study_ing wrote.

actuually goood point there moslem doc...im not sure if its a primary arising in lymph nodes of the salivary glands or wud u call it extranodal hopefully They wont ask us!!...HELP
