63yo pt with 6h history of slurred speech and right hand weakness. PMH: chronic hypertension and MI 2 years ago. Smokes 2 packs/day, alcohol occasionally. VS: BP 165/95

PE: Right hand weakness, mild motor aphasia NO sensory abnormalities.

Which is the most probable cause?

a. Large artery atherosclerosis

b. Cardiac embolus

c. Berry aneurism

d.Hypertension

e. Arterial dissection

That's the diagnosis. I wanted to bring this Q to te forum because it brings up te subject of lacunar infarcts. There are 4 syndromes.

Can you name and describe them?

with pathophysiology?

It is the small plaque that causes MI so a small carotid artery thromboembolism can cause a lacunar infarction too !
