Do we have to pose a transition statement for every part of H/O taking?...before taking past h/o, before taking family h/o before taking social h/o?? etc...

I think you have to say, but not every time. Just make it natural. Also don't fortget to ask if the patient has any question or concern. You can do that before PE or before you leave the room.

Thanks akachan! U r right I think as long as the transition is smooth, we need not always point out that we r moving frm one part to other part of h/0. ofcourse we have to use transition statements as a kind of permission while taking personal or sexual h/o n while doing PE. any more comments on this?
