A 20 yr old previously healthy woman presents with general malaise severe cough and breathlessness, which has not improved with a 7 day course of amoxycillin. There is nothing significant on examination. Chest x ray shows patchy shadow throughout lung feilds. The blood flim shows clumping of red cells with suggestion of cold agglutins.Most likely the causative organisms is mycobacterium tuberculosis mycoplasma p.carinii mixed growth of organisms legionella pneumphilia

true but, legionella is also "atypical" along with chamydia as i recall. cold agglutinins -think mycoplasma

X ray showing patchy shadows throughout the lung with a microbe... go for atypical pneumonia...
