a 43yo male Pt with CNS alterations, LAB: Pancytopenia. If you suspect leukemia, which type would it be?

Either AML or ALL... Think that both can produce pancytopenia...again is a wild guess based on age.

im a bit confused here--cant all leukemias theoretically cause pancytopenia?? but i guess id go for AML??

Pancytopenia is cause by acute leukemias only. Chronic leukemias show an elevated leukocyte count. So, yeah! It is AML or ALL. Now the other characteristic was CNS involvement. Which type or subtype of leukemia causes CNS most lykely?

going by the age...AML is more likely. ALL is more common in children. AML is common in adults. clinically it is almost impossible to differentiate only of the basis of clinical presentation.

I think it's ALL - though ALL isn't common at this age, CNS alterations are much more common for ALL than for AML.

wonderful an ! ya i just checked the notes and CNS involvement is most characteristic of M4 and M5 monocytic leukemia.

forgot that an has clinical experience as a hematologist besides huge theoretical knowledge. yes - AML 5.

Isn't AML Type 3 is the most common one

I don't think the exam will ask such question according to kaplan and all the tests I took. So relax and USMLE will not ask for teh subtype and even Internal medicine will not ask for subtype and only oncology board will ask for the subtypes. Thanks

another interesting point, though not high-yield..AML M5 can present as spinal cord compression, cauda equina syndrome...because of chloromas(granulocytic sarcoma) that can press on the spinal cord.
