How crucial for step 2 is ECG reading?should we know about all leads and what to expect in different abnormalities?

You must at least know the basics. Leads nice to know. But most of the time they just show lead II. Know arrythmias, AV blocks, PVCs, PACs.

I took the NBME. They asked a few questions including 1. acute pericarditis with ST elevation in all leads 2. different locations of MI, inferior wall MI, left MIi, anterior wall MI 3. Also they showed you hyperkalemia and ask you to give treatment 4. I think they have only 3-4 EKG questions 5. You should know paroxysmal AV supratachycardia. 6. I expect the NMBE ask any heart blocks i.e internmmittent heart blocks
