47 yr lady lives in a rural area and poor living conditions,has h/o breathlessness,orthoppnea and swollen legs.she is not a smoker but is an alcoholic,drinks 15 gm alcohol/day .She noticed a swelling on her anl=kle p/e : B.P 149/80,her short of breath is increasing when climbing upto exam couch .bilateral basal crackles CVS show diastolic murmur at apex .Tappering apex beat is present .What is diagonosis a- AR b-AS c-TR d-VSD The same question above what investigation would u like to do next day a- Chest X ray b- ECG c- Echo gramm d- US e- CTPA

Also this patient may have co-morbidity of a) alcoholic cardiomyopathy The medical treatment in aymptomatic pt is vasodilator But surgery is indicated for symptoms, LVD, and significant left ventricular dilation. **** Left ventricular dilation is probably more than 75 mm in diamenter (the normal End-diastolic ventricular diameter is probably 55 mm for a 75 kg man ) ???
