why does a patient who has been vaccinated with polio able to immune from neural polio, while actively replicating polio can still be found in the patients GI tract ?

If the patient was vaccinated with killed vaccine, dead viruses activated production IgM-IgG in the blood, but not IgA on intestinal epitelium. So virus can survive there, but when goes to blood - is getting killed. Also such patient will spread the virus. This is why live vaccine is better.

What vaccine is used now? I think in Kaplan I heard that first 2 are killed then live or something like it.

a little correction, killed virus is preferred over live attenuated virus cos it does not replicate in the intestines and so cannot be spread from person to person. in the US the killed vaccine is preferred cos of lawsuits arising from use of the live vaccine (sabin) which caused polio in the 60's and also contamination of some lots fo the vaccine in the past with papovirus sv40 which caused sarcoma in rodents. killed vaccine is given subcutaneously.

the live attenuated vaccine is given orally. it's advantage is that the virus replicates in the GIT and so the attenuated form of the polio is spread from person to person giving a milder illness and confering immunity to those affected. it is the form used in africa. the attenuated form is supposed to help eradicate the wild type which is still epidemic in africa and i think also in some parts of india.

Thank you tolito, now I understand. I found that from 2000 CDC resommends to use killed vaccine for USA. So all books before 2000 (most of mine) say that attenuated vacine need to be used.

hi ts97, i think for subjects like immuno, pharma, path, bioch you should have no book older than 2000 cos of all the new stuff going on around these specialty. my books are mostly 2002 and a few 2005 and i feel a bit jittery i may be quoting ancient stuff sometimes.
