transvenous cardiac pacing is a form of external and temporary pacing of the heart to tide over a crisis(as after an inferior wall MI with symptomatic heart block, drug overdose, etc), or to buy time till a permanent pacemaker is put in an acutely symptomatic patient...indications r asystole, symptomatic bradyarrhythmias and tacharrhythmias... the electrode is inserted into the right ventricle thro either the lt subclavian/int jugular/femoral/brachial vein...the generator(basically a battery) is outside and u can set the stimulation and sensitivity threshholds...

The indication for biventricular pacing is CHF with QRS intervals greater than 120 ms !!!!! Transvenous pacing is seldom used in the real world. In the emergency world and out in the world, EMS used external pacing !! The door-to-ballon or door-to-cath lab is 90 min. Transverse pacing may be used in the ER only if the cardiologist may be late in hours before going to the OR for a permanent pacing !
