Isnt it that in sec hyperparathyroidism, there is decreased Calcium and decreased plasma phosphate? Why C?

They cannot both decrease. CRF-->decreased phosphate excretion-->increased serum phosphate-->decreased serum Ca...> hyperthyroidism.

The answer is A. Renal Failure.... cannot utilize Vitamin D (25-O etc,,,) cannot absorb calcium and PTH is decreased because it needs vitamin D.

it DEFINITELY is not A. there is secondary hyperPTH in renal disease.
