portocaval shunt????

5-portocaval shunt.

portocaval shunt alongwith beta blockers is better than sclerotherapy to prevent rebleed but also has more incidence of encephalopathy and does not actually result in decrease in mortality. and it also requires periodic monitoring with doppler or hepatic venogram to check for stenosis and thrombosis of the stents. thus it is used mainly in the event of recurrent bleeds i.e. >2 bleeds and failed sclerotherapy and pharmacology.

PORTOCAVAL SHUNTING IS MOST APPROPRIATE IN CASE OF RECURRENCE AS THE OTHER CHOICES ARE USED AS FIRST LINE OF MANAGEMENT.
