It is also this that generates a lot of new enthusiasm about anti-angiogenesis drugs along with anti-epitelial growth factors to fight against cancers along with the new modulators to alter the receptor site or to occupy the receptor cell or to block the receptor cell at the membrance level to stop the growth of cancer. If cancer stops growing, then there is no metastais. It is based on different level of blocking the growth of new cancer cells that we are able to "control" a lot of cancer such as chronic myelo leuckemia.

Look at prostate cancer, it is the mass effects and also the metastasis of cancer cells at distant site to cause mass effects to destroy normal growth. You don't need metastasis but local extension of lung cancer through mass effect to kill you. Did metastatic breast cancer which exert mass effects or the metastasis of breast cancer that kill you ?

i think paraneoplastic syndrome-cause metastases themselves don't kill-but what they produce-i.e.-different substances,hormones-cah actually kill. what's the right answer anyway?
