hello al... pl pl tel d ans n also y u chose it...

a 70 yr old man comes to u complainin of dec in force of calibre of his urinary stream.on examinatio he has a slightly enlarged prostste.... assuming his problem to b bph optimal drug therapy shud involve:

a)clonidine

doxazosin

finasteride

saw palmetto

sildenafil......??

nw d ans is (sry to disclose it).... b doxazosin... my doubt y not c??? optimal tmt???hence b? n cos v r presuming d dignosis.. pl. guide. isn't d q slightly wrong in wording????

doxazocin is alpha blocker so it will improve the poor stream in bph.more precicely its alpha1 blocker so will dec the sphincteric tone finesteride is 5 alpha reductase inhibitor more commonly used in prostatic ca

its a naturistic herb i believe though to help in bph

in classical medicine its the alpha 1 blockers (eg.doxazocin, terazocin)

in complimentary medicine its saw palmetto

finasteride inhibits 5 alpha reductase--so the more potent dihydrotestosterone is not available

clonidine acts centrally and is an alpha 2 agonist with multiple uses

sildenafil has to do with inhibiting GMP and NO2 becomes available

**with alpha 1 blocks postural hypotension should at least be somewhere in the crevices of the temporal lobe!
