You are considering treating a 21-year-old female with a history of anorexia nervosa and depression with an antidepressant. Which one of the following should you avoid? 1) desipramine 2) citalopram 3) venlafaxine 4) bupropion 5) phenelzine

Bupropione. The reason is: eating disorders have in their pathogenesis seizure activity, and bupropione(Wellbutrin )is containdicated in seizures(and eating disoders, accordingly)

the way i approached it was to r/o options that are used in treatment i.e. 2 & 3. after that i wasnt too sure. ssri's increase serotonin levels in the synapse and can be used for treatment of depression and maoi's by inhibiting breakdown of these neurotransnitters may still be used. desipramine has the action of increasing norepinephrine and to a moderate extent serotonin too, so that leaves me with bupropion. i'm curious to know the right answer and the explanation!

Common sense goes a long , long way ! You don't need to be a doctor to know the answer. And Bupropion is the wrong answer ! Bupropion is a nicotine cessation drug and can cause weight gain. The secretary even knows about this ! So D is wrong The correct answer is E

5. Phenelzine If anorexia nervosa in this patient is subtype binge - eating/purging, she is likely to eat many foods that can interact with MAOI's.

i'd say 5-phenelzine- with bupropion it's clear-may cause weght gain.but it's not that life-threatening. but with drug-food interaction if you put her on MAOI-it's pretty dengerous. what's the right answer, determine?
