A previously healthy, 68-year-old woman develops auditory hallucinations. She cannot provide many details but believes her mother is speaking to her. She has difficulty cooperating during the interview and physical examination, which is unremarkable. Which one of the following is the most likely diagnosis? <?xml:namespace prefix = o ns = "urn:schemas-microsoft-com:office:office" />



1) complex partial seizures

2) Alzheimer's disease

3) adverse medication effect

4) hyperthyroidism

5) peduncular hallucinosis

I think, 1.Peduncular hallucinosis=visual hallucinations.Alzheimers dis-possible, but they would have mentioned cognitive impairment.Adverse med effect-possible, except we assume she's healthy and not on meds.Hyperthyroidism-with hallucinations?Complex partial seizure-remains.

previous healthy= no medication (code word)=no medication side-effects

hyperthyrodism= weight loss, tachycardia, atypical presentation in elderly, no seizure

Alzheimer's=slow progressive, cognitive impairment

I asked to one of my psychiatrist friend, she says it should be peduncular hallucinosis.

However, in peduncular hallucinosis, there are usually visual hal!!??

I checked that out. Both auditory and visual hallucination can be associated with peduncular hallucinosis and I thank everyone to check that out !

I google everything and they came up with visual and then I type in auditory and said both visual and auditory can be associated. Thanks "determine"

I checked- it says-auditory halluciantions "also have been reported"...
I'd say, it's auditory aura, then seizures (which are easy to miss if You're not there-may be she's just staring on smth for couple of minutes, and then it's gone), then she's got the postictal slight confusion(that's why she has trouble cooperating).
But I cannot be sure. What do you, guys, think?

The ANSWER IS A. Yes A

Partial seizure=electical stimuli involved only part of the brain

Complex partial seizure= involved unconsicousness

Symptoms can involve movement, loss of consciousness, visual and auditory hallucination

The answer is COMPLEX PARTIAL SEIZURE.

Do you guys like me (I get tired every couple hours 3-4 hours intensive study and have to sleep for 10-15 minutes. But now I drink some coffee. I hope I am not getting complex partial seizure--passing out, auditory hallicatination. LOL !!
