6. A 55-year-old man is hospitalized after having a stroke. The MRI of the head is obtained 3 days later. The patient most likely has which of the following signs on the right?






A) Homonymous hemianopia and numbness of the face

B) Weakness of the upper limb and aphasia

C) Weakness of the upper and lower limbs

D) Weakness of the face, upper and lower limbs

E) Weakness of the face and dysarthria

Agree MCA distribution.
MCA would not however affect lower limbs (that would be ACA),
so i think its B. (Aphasia because of temporal lobe involvement).

I am very terrified with neuroscience. I read many times but then forgot.

Middle Cerebral Artery:

Spastic paresis of contralateral lower face and upper limb

Anesthesia of contralateral face and upper limb

Aphasia

Contralateral superior quadrantanopsia

other: left-sided neglect (if dominant afftected)

Lower Limb is ACA. (Medial)
Everything else is MCA. (Lateral)

Dysarthria is an acquired neurological disorder of speech caused by damage to cranial nerves or the frontal lobe (motor cortex). Dysarthria is completely a motor problem. Symptoms may include slurred speech, paralysis of the speech musculator, or swallowing problems. One side of the body is usually paralized.

What is wrong with A?!!

Hemianaopia should require occipital lobe involvement and numbness shoudl require parietal or thalamic involvement.
The answer is B and the aphasia is likely to be only Broca's.

B. exclude choices with defects in u and lower limbs together in this case coz of different vascular supply. B goes with what is shown in Dr virgo's homonculus
