A 57-year-old woman is receiving total parenteral nutrition and pre-operative radiotherapy for obstructive esophageal carcinoma. She becomes lethargic and demonstrates bizarre behavior. Urine output is 3400 mL/24 hours. Let's dissect this question and ask WHY ????? WHY ???????? An esophegeal carcinoma patient= must be starving, nothing to eat pre-radiation= ready for syrgery and poor intake TPN= high solute, hyperglycemia TPN means a lot of highly concentrated nutients 3400 ml/24 hour urine output= a lot of water get out along with some solutes patient is pouring out large amount of urine (3400 ml of water with a lot of sodium) Electrolytesimbalance which one? A sudden shift of electrolyes and water causes what ? hahahh Think Hyperglycemia cause pseudohyponatremia
