In which one of the following situations would it be unethical for the psychiatrist to break confidentiality and notify the authorities? <?xml:namespace prefix = o ns = "urn:schemas-microsoft-com:office:office" />



1) The psychiatrist believes that the patient will probably commit murder.

2) The psychiatrist believes that the patient is actively suicidal.

3) The patient has severely impaired judgement and is in a responsible position, e.g. an airline pilot.

4) The patient has aggressive fantasies.

5) The patient has taken a significant overdose

perhaps the patient likes the idea of violence..perhaps he says that he wud like to blow up the world or something...

Why not 1?? Physcian has the legal obligation to at least try to notify the potential victim.

Should suicide be notified? Needn't the patient just be hospitalised and treated?

The correct answer would be Whenthe patient has taken an overdose. Only in that case it would be unethical to break patient confidentiality

i guess others are not at risk in this situation..certainly deserves a thought..

I say 4, not 5, cause fantasies are not the same thing as a plan to harm self or others, or an actuall attempt to harm self or others
re #1 old case in Cali, Tarasoff v. UC Irvine I think, guy goes to student counseling center, tells the intern he is going to kill his former girlfriend, and on what day and how. Student tells supervisor, supervisor says you can't call police or tell the girl, girl dies, school and intern are sued and lose

i think it is 5

the rest puts others at risk including agressive fantasies since pt may " hear commands" asking him to act on it

sucidal i guess is a strong contender here but if you haev a sucidal pt you act to save the pt.forcibly admitting him if need be so i guess confidentiality is out there.

the only odd one here seems to be a pt who has already comitted the act of attempting sucide.
