A 20-year-old man has been involved in a motor vehicle collision. He has obvious blunt chest and abdominal injuries, but the striking feature is his marked restlessness. Which one of the following is the most important initial step to take?<?xml:namespace prefix = o ns = "urn:schemas-microsoft-com:office:office" />

1) Check for causes of hypoxia.

2) Give him morphine.

3) Check him for head injury.

4) Give him a sedative.

5) Catheterize his bladder.

I think 1 because marked restlessness is a result of brain hypoxia.Especially he has chest trauma. But I'm not sure. I have to have a reliable explanation to be sure.

The marked restlessness might very well be attributable to head injury leading to brain hypoxia.In an accident such as this...rather any trauma situation isn't a thorough head to toe exam mandatory?So,in my opinion a head injury needs to be ruled out.I look forward to some more input on this.

What do you check: fixed dialted pupils, blood in the external canal, clear fluid from the nose !!!

blunt chest trauma , marked restlessness ...no sign of severe deceleration injury / head trauma so answer might be to check for the cause of HYPOXIA due to blunt chest trauma may be.

I really prefer American exam question because they give enough information to dissect the question and give a definite answers.

I took British, American and Canadian exam.

I prefer NOT to say here which one I prefer.

American exams are two days long and very exhausting but there is just not enough information here.

I took ACLS, ATLS and they want you to start with ABCN, N is neurological, A is airway. So check for hypoxia is the Canadian exam they want you to know.

This is based on Canadian standard and not American standard.

He most probably has a full bladder causing restlessness! and 5) catheterizing the bladder could help him loads!!:o)
However, the most 'important' initial step....i go for 3 too. Causes of hypoxia are far too many and checking for all those means too many precious moments taken. IMO, checking for Head Injury is important and appropriate as initial step after any MVA.
