chlamydia. it is too small to be seen on gram stain a diagnosis of NGU is made when smear shows PMN and not intracellular or extraC GN. GN will not reveal chlam. the most common cause of NGU is chlam.

This is a high yield. Genitourinary infection, urinary neutrophils, sexually active persons, but no gram stain evidence, we should think about Clamydia

It is not too small to stain - it just doesn't stain. Also, DNA amplification is the current method that is used in huge numbers of laboratories to diagnose Chlamydia. We don't get any gram stains on urethral discharges from males any more at all from our ER. There are other methods also, but the latter is rapidly replacing the other methods. The DNA amplification sample usually tests for GC and Chlamydia with one sample. Please realize, as previously mentioned by myself in another thread, that this is NOT a stat test due to the cost of running it. Our lab does huge numbers of these M-F. They are not run on the weekends and are batch tested by day shift technologists. Also realize that DNA amplification testing is not always accepted by courts in child abuse cases, so chlamydia and gonorrhoeae cultures still need to be taken in these instances. Make sure you check with your lab regarding proper transport media for these specimens as most places send them out. If you remember nothing else about micro please remember this: WHEN IN DOUBT ABOUT HOW TO ORDER OR COLLECT A SPECIMEN FOR MICROBIOLOGY, PICK UP THE PHONE AND CALL THE MICRO LAB FIRST!
