a 45yr old presents with symptoms and signs of Multiple sclerosis. if confirmed the following will result in 1. bitemporal hemianopia 2. lost of blink reflex 3. loss of light reflex 4. total blindness 5. internuclear ophthalmoplegia

FIRST i made the question up so these are my answers from reading kaplan and MRS neuroanat answer is 3. the pathway via optical nerve is lost cos it is the only nerve affected by MS (kaplan info) bitemporal hemianopia in pituitary tumor loss of blink reflex involves V and VII total blindness ... well i know they have central scotomata and not total blindness.. correct me if i am wrong internuclear ophthalmoplegia has to do with cr VI, MLF, and III

the blindeness is usually unilateral.the ligt reflex will be intact on one eye and on the affected side only the direct light reflex is lost. Golijan said they have bilateral internuclear ophtalmoplagia b/c of the demyelination of medial longitudinal fasciculus.

mmm thanks mitty. i got only half information obviously. i can understand the loss of the direct while indirect is intact in th affected eye cos indirect is via III. good information about the internuclear ophthalmoplegia.

so it is loss of light reflex in one eye and bilateral internuclear ophthalmplegia..

Question stem is incomplete... 2 common/classical features of MS are 1. Optic neuritis 2. INO others Unilateral ataxia, unilateral motor symptoms, unilat sensory symtpoamns, myelopathy, bladder, trig neuralgia, etc
