In UW, they say that we need to auscultate the heart when the patient is in lying down position and then ask the patient to sit up to ausculte again My question: Can we auscultate the heart only in lying down position?

I would say from my own experience and infromation, auscultate the heart in patient liying with his head end at 45 degrees.. it is convenient for JVP as well....then ask the patient to lean forward for murmurs.... thats what one does in UK and is written in reputed clinical exam books as well.. not yet started prep for CS, so unsure of US standards

You need to follow the book to exam the heart from inspection, palpation, ausc. etc. You need to exam and listen in different position, patient lying down and turn to the left, patient standing up also tell the patient to do valvular, hand strength, from standing to squat, from squat to standing Why? They will give you the aortic stenosis and also the IHOC cases and you need to do all the different positions. Remember you need the different position and the change of position too good luck

Hand grip, from squat to standing, left side lying down etc.

should we do all this sitting and lying down positions in every body with every complain or we do it only in cardiac cases? and if we do it only in cardiac cases for other cases which one is preferred>? i was wondering not to lye down the patient at all if the patient doesnt need abdominal exam and listen to the heart and lung in sitting position in front in order to save some time.is this correct?

Do the vasalva maneuver only if needed. The case will point to it. But do examine the heart lying down then sit the patient examine sitting up and then you do respiratory.
