hi,
iam reading kaplan notes for path and its given, there is p-ANCA association with polyarteritis nodosa.
robbins says that there is no association and p-NCA is associated with microscopic polyangitis and churg strauss..
help me regarding the correct answer!

Nothing is more authoritative for pathology than 'Robbins'!
by the way, Goljan also says (incorrectly) that there is p-ANCA association with polyarteritis nodosa.

p anca doesnt associated with pan

i've had this doubt for a while, every source but robbins says PAN is associated with p-anca. i trust robbins over every thing else but i wonder what the boards thinks on it.

OK, I'll check in clinical textbooks.
Board thinks what is reality and reality can be only one - either PAN is associated with p-anca or not.

Laboratory Findings

Most patients with polyarteritis nodosa have a slight anemia, and leukocytosis is common. The ESR is often (but not always) strikingly elevated but, in general, serologic tests—particularly autoantibodies—are of little help in making the diagnosis. Patients with classic polyarteritis nodosa are ANCA-negative and may have low titers of rheumatoid factor or antinuclear antibodies, both of which are nonspecific findings. In patients with polyarteritis nodosa, appropriate serologic tests for active hepatitis B infection must be performed.

guys..i guess there shouldnt be much debate on P-ANCA being +ve in PAN ...its only 30% cases so -ve test does not exclude the dx..secondly BANG!!! both p-ANCA and C-ANCA can be +ve in PAN (source: emedicine.com) ...so i think we shouldnt make much fuss about this ...i would follow kaplan and brs in this matter
