If it's related to your differential I would touch it (WITH GLOVE and carefully not to cause discomfort to the patient) because you have to describe it as detailed as you can in the patient note. If not, just look at it and mention it briefly in the patient note.

thx claudia....can tell me briefly how can i describe a rash..i don think that i ve ever seen a derma pacient note(considering that is a derma case or a les or..whatever)
