A 22-year-old woman presents with a 1-week history of mild lower abdominal pain and a yellowish vaginal discharge. DNA probe testing for GC/Chlamydia is much more common in our lab now than actual culture for GC. There are still some docs that are holdouts and won't order the DNA probe testing. There are also some legal issues. An abuse case of an underage child requires culture instead of DNA probe testing. I'm not sure why - apparently courts haven't caught up with new technology fully yet. Gram negative intracellular (diplo) cocci on a MALE urethral swab is diagnostic for gonorrhoeae and can be reported directly to our state health department. However, the docs usually just get a DNA probe from the guys and that's it. Gram negative cocci on a FEMALE specimen can be suspicious but is not diagnostic and must be confirmed by culture or DNA probe. Certain organisms in the female genital tract can present as gram negative cocci. Our emergency room routinely runs this "crotch" battery (as we techs call them....): wet prep for trichomonas, gram stain of exudate and DNA probe for GC/Chlamydia. The latter is batched and not run STAT as the test is expensive and does not lend itself to stat testing.
