When the patient is having Ventricular Fibrillation, we're defibrillating patient by hitting them with the electricity (200J, 200J, 200J, then 300, then 360J)-just like You've seen in the ER.
When the patient is having other acute rhythm disturbances(tachycardias) eg VT, atrial flutter, and becomes unstable (low BP, altered level of consciesness, nonresponsive)we're doing cardioversion by delivering the electric charge of lesser strength(corresponding to the protocols).
Sometimes we do synchronized cardioversion-when the patient has acute rhythm dysturbances, was tried to be managed with meds, but unsuccessful ( the patient is usually hemodynamically stable) then we deliver electrical charge synchronized to the patient's EKG-R wave. We don't want to deliver it on Twave because it can create instant VF and get things complicated.
