A 55-year-old woman has a bloody discharge from her left breast. A mammogram discloses a cluster of microcalcifications 3 cm beneath her left nipple. The next step in her evaluation is (A) cytologic evaluation of the nipple discharge (B) fine-needle aspiration under radiologic guidance (C) CT of the breast and axillary nodes (D) open biopsy of the left breast (E) segmental mastectomy

Papiloma intraductal is main cause of breast discharge discharge. This is a risk for malignancy especially because the discharge is bloody and this strange forms of microcalcification suggest mailgnancy. Fine needle aspirations gives an inapropriate material in 25% of the cases. With palpable mass, it is suggested surgical biopsy.
