I'll go with E. You must find out what she means by abusing before making any suggestions. It's like those questions where patients coming in and say they have problems with their "sexual relationship" - you must find out what exactly they mean by "sexual relationship" first!

I'd say its B. Even though spousal abuse is not a reportable issue, the woman should be aware of her situation in order to report it herself, or at least take some action... so I think making her concious about if she is safe in her home would be the best answer.
Besides, no matter WHY is the spouse abusing her, it is wrong.

Guys, did you visit a PCP office in the USA? Among firsts questions ( NO MATTER what is your problem) is this "Do you fell safe at home?" even not the abuse is the problem. You have to be sure about the SAFETY of your patient!

Dear, though the pt has alcohol dependence,but before meeting doctor it's not said that she has already taken alcohol...E will be a better option.

use this common rule, always ask why? on the test aswering questions, and with a patient. Listen more , speak less

I think A,Cand D can be excluded because they are either too brisk or make the patient guilty about her behaviour. Out of the other 2, b seems a better response since physicians primary duty is to protect the patient.
