B. Because the first thing to do is make sure the patient's life is not in danger by her returning to her home. Everything else, such as why the abuse is occuring or how to stop it, is secondary.

B is a strong answer-{ you look out for her welfare objectively} I think its unfair to blame only alcohol

2 things are good: to make her talk .. and find out why is he abusing her .. if its related or not to her situation.
let see the answer ,,

I STRONGLY believe it is B...but I have no factual basis whatsoever other then the well-being of the patient a primary concern.

If you think E well you might get some answers but then she goes home and BAM! knocked upside the head and unconscious the moment she gets home! How is that being a good doctor?!

I AM SURE IT IS B. THE SAFETY OF THE PATIENT IS UR FIRST PRIORITY. EVERYTHIN I REPEAT EVERYTHING ELSE IS UNIMPORTANT

you can't just assume everything on one line of the pt...you need more info...a is not right b/c you can't jump to correcting a situation w/o knowing all the details. I would go with E...

In the Kaplan Cd's, the psych professor says that always get more info and if in doubt about an answer, always pick the one with the pt. talking rather than the doctor. More info is always good

guys if u want to score a point in usmle step 1 ......for god sake answer this one as E.......never forget this basic .....allow the patient to talk more.......

B was a real distractor......the patient's safety is important but here she is not in a state of commiting suicide.....or has not been threatened by the husband

I think B is the correct answer. Patient's safety is a priority.

the answer is B. i saw similar question in BRS 4th edition chapter 20 p 192 ( aggression and abuse). fadem says the most important thing is to protect the patient's safety. D,E appear accusatory as though she were to blame for her problem and they are not appropriate.

The answer is B.
This is what they teach here in American Medical schools. NEVER choose option E in this case coz you come across as blaming the patient, D is the same,, nobody deserves to be abused even if they are alcoholics. Now after making sure the patient is safe, then you can move towards step 2 which is addressing risky behaviors that compound or expose the patient to abuse. But your intitial response is to make sure the patient is safe.
I am sure of this answer.
