I think it's A. increase pulmonary artery pressure she has obstructive sleep apneea which leads to hypoxia and finaly to pulmonary hipertension and cor pulmonale

yes it is A but way swollen ankles i think by apnea there is little perfusion to the kidnythat will release its renin
