In GVH the donor's CD8 cells play the major role in destroying the recipient's cells. First activated helper T cells secrete IL-2, which stimulates cytotoxic T cell to form a clone of activated cytotoxic T cells. This cells have 3 mechanisms: perforins, granzymes and Fas-Fas ligand (FasL) interaction. Perforins form holes in the membrane so cells die, it also allows granzymes to enter. Granzymes are proteases but also activates caspases that initiate apoptosis. When Fas-FasL interact, apoptosis of the target cells occurs.
