A 33-year-old man of Hispanic descent comes to the emergency department with his 6-year-old daughter. The patient, who speaks only Spanish, appears in mild pain and says "dolor" while pointing to his abdomen. His daughter speaks both English and Spanish. The physician does not speak Spanish. Which of the following initial actions by the physician is most appropriate? A. Ask a hospital housekeeper who speaks Spanish to interpret for the patient B. Ask the patient's daughter to serve as an interpreter between the patient and the physician C. Have a professional interpreter present while the patient is being examined D. Use a medical English-Spanish dictionary to aid in understanding the patient E. Use pictures to aid in understanding the patient

In a perfect world its answer-c {also for usmle also} But whats in the text book and what happens in the real world is usually different. A housekeeper is a candidate in something like acute appendicitis.

In emergency cases do we have professional interpreters available?

I am guessing B is wrong because she is "only 6" and is not considered "smart enough" to translate medical talk.

Body or picture language is used when the patient cannot speak English, especially in emergency cases,

theese questions are so STUPID!!! I hope I won't get any of them in my exam. The phisician can think for himself. If the girl can't translate accurately, he SEES that and can ask for the janitor's help. so f*** off USMLE w/ your stupid questions!!! why does it have to be only one of theese stupid answers? I dunno which one of those answers are true!!!!!

Chill...do what I do. Just pick the LAMEST answer. What USMLE thinks is right does not necessarily translate to the real world. Ideally, the correct answer would be C because it is the "right" thing to do no matter how unpractical it may seem.

Dear ManuNastai I read what you wrote. That is exactly what I feel too. Please please any one of you.... guys.... do suggest a source where I can find plenty of such ehics/physician patient relationshop questions so that I can practice on as many as those and probably raise my spirit about it... otherwise it is making me feel down everytime I miss these questions......Please respond

Actually, this questions are funny to solve - you don't need to recall facts from your memory (as, for example, when solving pharmacology questions), you just need to understand 'the soul of the US medical practice' . Yes, you should know some facts about ethics and legal medicine in US, but after you have read them once, you'll never forget any of them, because they are about every-day life common sense, something very close to our mind, not about, for example, how pioglitazone works. There are 3 great books I have read for Behavioral Science: 1. 'Kaplan Lecture Notes: Behavioral Science' 2. 'BRS Behavioral Science', by Fadem 3. 'Rapid Review: Behavioral Science', by Stevens I have had complete understanding of this 'soul of the US medical practice' only after reading all 3 resources - they supplement each other.

It's not really a dumb question; just one that requires a little common sense. According to the question stem, the patient is in 'mild pain'. There does not appear to be any reason not to wait for an interpreter. The housekeeper might not be able to comprehend your medical terms and the daughter certainly isn't mature enough to do that either. Using pictures or a medical spanish-english dictionary might confuse u and the patient more.

Well, i just read that for USMLE the correct answer is always the ideal scenario. it does not have to be practical. for example, it may not appear practical for a professional interpreter to be in the emergency but that is the ideal thing to do. we are further assisted by the comment that he is in mild pain. so i now go confidently with C.

yes, huzefa is right. I tend to skip those words that give away the answer 'cause I'm very impulsive. if you saw that 'mild' word you can think that the choice with the interpreter is the answer but I still think they're STUPID and I wait for th americans to give me the MD title so I can show them how good I am in speaking with children and treating their parents

USMLE wants you to know what some spanish people ( they have a lot of spanish people here!) would do for them parents or relatives! The spanish are like romanian people- don't tell the patient the truth, so his daughter will not translate everything!
