1. It really depends on in which aspect you really need to boost your credentials. You can not improve your USMLE scores, but you may try to get USCE, good LORs, and research experiences, etc.

2. During your interviews, they may hint your weakness, so try to make up for it.

3. I graduate 8 years ago, and matched into a top anesthesiology program. And I know some ~ 50 year old lady matched into a pretty decent neurology program about 2 years ago. So do not give it up. Just decorate yourself in a positive way.

4. Good luck!

mghanes, congratulations on your success. Though, to be honest, 8 years look much more attractive than ">12 years". The truth is : there are a lot of recent, smart graduates out there, and they are much more attractive to the programs than old smart graduates. Period. It is still possible to make it. ( I made it with 15 interviews and 3 prematch offers. I had only 3 invites from the University programs though. The keys are-- good scores ( they are far from being enough by itself because a lot of people have good scores, but if you do not have them-- the chance is much lower);no visa problems, no gaps from medical field ( say, after graduation you were practicing physician all those years), US clinical experience. USCE is of a tremendous importance for an OLD graduate: you kind of showing that you are up to date in a current hospital setting, and you get recommendation letters from American doctors. During a couple of my interviews those doctors's names were recognized. Boy, it changes everything right away: they look at you like you gained some "weight" from now own. Another thing is, you have to sound confident during interview and be able to talk about everything. The saddest thing to me is that a lot of programs have 2,3,5, 10 year barrier, so you are not even given a chance...I admit, 50yo can get into Residency but that is handful number of people with Degree and years of research... A second look interview helped one of my friends during this year match.. She had only one interview ( but PD did not know that, right? wink, wink). So she told them how much she loved the program and asked for a second look. The result-- they offered her a prematch while she was there for a second look. And it is OK program. Not to die for, but pretty good for low scores and a big gap in practice. Once you do something in the hospital you get to know people. An externship is the best. But Anything better than nothing. And if you are smart, helpful and nice, they can even help you. Another friend of mine found a position in research by pure chance 3 years ago. The PD of that very competitive program found that guy to be really good. So, when the match season came two years ago he said:" Look, you have no chance in THIS program but I am going to help you to find a spot." And he did. Besides a great LOR he personally called PDs of less competitive programs , and the guy got his slot. So, the bottom line is: knock, and the door will open, but it is likely to be frustrating, and "up and down" experience. Not very optimistic, I know. But it is better to know your chances in advance than to land on your bottom and go through depression like I did.

Thank you for that much information. The sad part is, if you are an img it is almost impossible to know physicians in thie new country , what is on egoing to do? Even when we try for observerships, most people dont even acknowledge youemail!

Anyone who working in hospital will have some help. Do not have to be a doctor. Try to know every people from your country or even in your community as you can. Maybe there is one PD in your neighber. As for myself. I graduated 11 years ago, no USCE, no US doctor lor and applied late in nov, but a friend of my sister in law works in a community hospital as cath room technician, so she send my resume to PD and I got an interview chance before my CS result and USMLE certificate. Now I had matched with that only interview.

rashmi, you do not have to be that cynical, you know. You are the best by your own standards, of cause, but people in the new country do not know anything about you. And, simple psychology applied, they would rather go with a " sounds-good- known" than with a " sounds-good- totally strange." I could not believe to what degree USA is a country of REFERENCES. They count letters of recommendations, they read them, and yes, they listen to the people who suggest candidates. That does not mean you have any guaranty because you still have to be qualified. But everything else equal-- a word can help you. To every place I went for the interview, there were always hugging and kissing among already a resident and a candidate who went to the same college together or was from the more rare country than India on the market. Do not tell me that they would not say a word to PD about how wonderful "such and such " is. Do not want to offend anybody personally but some PD like graduates from particular countries and such preference you can see from the residents's last names on the program website. So, you just do not apply there because the chance is zero: you do not belong there. Amygdalaa, if you are ignored by e-mail, then you call. If nobody talks to you, you show up in person as charming and as devoted to your profession as you possibly can be. One of my friend started with volonteering nights in ER as a scriber, at the same time she found a place in a research. At the same time she worked for a private physician, AND she has kids too, AND there was nobody to speak for her... When you truly want something-- you will find it. If you sit and wait that somebody will "acknowledge" you... the outcome can vary. Of cause, if one graduated yesterday from a well-known foreign college, has LOR from recognized physicians in his country, has a couple of publications AND even has undergraduate rotations in USA-- that is the different story. There are the universities for those people with the following words on their websites: "The USCE is not necessary". Everybody else ( w/o credencials that make them special )goes down hill. Yes, they end up somewhere but do not tell me that the majority of FMG will have even their 4th choice. Yes, nice exeptions happen ... we would not call them exeptions if they were that comforting common, would we?

i agree with cs obsessed....you have to have it in you for a word to help you .....and if we indians only talk this way about our country, how can we stop others????disapproval

Hi CSobsessed,

You are right. Afetr persistence and many phone calls, I think I will soon be doing an observership in a university, I hope I will, since they invited me over and showed me around and seemed willing.

As to 'things workign out when you know people', no country is an exception.

Amygdalaa, that is wonderful!!! And very timely too, because "New September" will come fast. It took forever for my certificate to come, so I had an opportunity for externship only from September which means that my LORs were written much later than I wanted them , I also had to push my interviews. So, make spring and summer as useful as possible. Good luck!!!
