hi friends, to apply for the NOC or statement of need from India(for J1), we need to submit "written assurance in the prescribed form" along with other documents. can anyone tell me where can i find it? what should we write in the letter? where is the 'prescribed form'? thanks for the help and good luck to all

Contact the office in Delhi, if you are an Indian Give them 1000 Rs, they will do it for you.

assuming by your name kavya that you are an Indian - go to ministry of Health and Family Welfare at Nirman Bhawan, New Delhi and take with you a copy of your passport, and a copy of the contract letter. the written assurance form's contents can be found at the following link: http://www.indiacgny.org/customimages/Statement-of-Needw.jpg take that typed on a sheet of paper too. should get it easily....hopefully. i'll also go there once i get my contract letter...let'see if i bump into you there... ---------- No Objection Certificate' for Indian Doctors under going higher training/studies in the USA Consulate General of India (in Washington DC only) is authorized to issue certificate for residency training in the medical/medical speciality for a period of one year or more and for any medical speciality where the duration of training is less than one year. ( you will need to renew every year) For areas involving more than one year of training the Ministry of Health & Family Welfare may be approached directly at the following address for necessary authorization(seems like the better option- since you keep the original and send a copy of the same every year to ECFMG for the renewal) (current as of March 2006) Ms. Sumitra at the office of Ms. Anita Puri Under Secretary Office of International Cooperation Rm Number 752-A Ministry Of Health & Family Welfare Nirman Bhavan, New Delhi 110 011 Phone Number (011 91 from US) 0091 11 2306 1945 The Consulate will issue the certificate for a period of one year initially. A written assurance in the prescribed form, undertaking to return to India at the end of the training must be submitted. This has to be sent directly to the Ministry of Health & Family Welfare at the address given above, a copy endorsed to this Embassy along with the application. Certificate for subsequent years can be renewed only after written assurance has been submitted and on production of a letter from the Ministry of Health directly sent to the Applicant, stating that the written assurance has been accepted. DOCUMENTS REQUIRED FOR ISSUE OF 'NO OBJECTION CERTIFICATE': A Copy of the Contract with the Hospital/Institute on the standard Agreement Form with a letter from the Program Director on the official letterhead with seal and contact telephone number. A U.S. stamped self-addressed envelope. Doctors can specialize in any number of fields and the restrictions on the listed fields earlier have now been removed.
