A 56-year-old male with a history of heavy alcohol
use presents to the ED complaining of vomiting blood
for several hours. Vital signs are BP of 90/60, pulse of
110, respiratory rate of 16, and temperature of 98°F.
Primary resuscitative measures are begun, and placement
of an NGT shows active bright red bleeding.
Which of the following is the treatment of choice?

(A) Tamponade with a Sengstaken-Blakemore tube

(B) Therapeutic upper GI endoscopy

(C) Octreotide infusion

(D) Vasopressin infusion

(E) Immediate referral for surgical intervention

I think he is suffering from Bleeding esophageal varices....All of them can be used as treatment choices but i think trying Therapeutic upper GI endoscopy first would be better ..
Emergency surgery may be necessary to treat patients for whom other treatment fails

When you have an upper GI bleeding the first step (obviously not life threat) is to see where the bleed comes from, so the answer has to be B

all setups may not have every thing available.
I would just push in a Sengstaken-Blakemore tube
and then prepare for the endoscopy.

Bleedin from VE (most likely) = tto
Unstable hemodinamically = IV fluids, blood transfusion, upper endoscophy
Stable hemodinamically = TIPS
