A 34-year-old Caucasian woman presents to the emergency department with thoughts of suicide. She states that she was feeling "fine" until a week ago, when she began to feel very depressed. In addition, she states that she has not slept very much in the past few days and she has been experiencing rapidly shifting extremes of mood, one day feeling "on top of the world" and the next feeling "tired and down in the dumps." She claims that she has thoughts of jumping in front of a train to kill herself. When asked about illicit drug use, she becomes rather indignant, stating that she is being accused of being a drug addict. A urine drug screen is positive for cocaine metabolites. Which of the following is the most likely diagnosis?

A. Bipolar disorder, manic
B. Dysthymic disorder
C. Histrionic personality disorder
D. Major depressive disorder
E. Substance-induced mood disorder
