A 49-yr old previously healthy woman reports having suddenly lost consciousness four times in the past 6 months. In three instances, she was unconscious for only a few minutes. After the fourth episode 1 month ago, she was unconscious for 6 hours and had weakness in her left arm and difficulty speaking. On physical examination, she is afebrile and her blood pressure is normal. She has good carotid pulses with no bruits. which of the following cardiac lesions is most likely present?

A. Pericardial effusion
B. Left atrial myxoma

atrial fibrillation causes repeated thrombosis. of above choices, D is most likely to result in atrial fibrillation.

Yeah guys you have a point with atrial fibrillation, but correct me if Im wrong, but isn't cardiac embolus produce hemorrhagic infarcts in the brain? This patient has more transient symptoms, which means she has something reversible.. Having my doubts here... I would really appreciate if any of you could clear this....

it should be E cause transient ischemic attacks are caused by little thrombi that lodge themselves in branches of the int carotid. and since the coronary arts. are the main source for atherosclerois, they should have the most potential for thrombosis

i thought it was D also, but correct answer is B....atrial myxoma can have a "ball-valve" effect that intermittently occludes the mitral valve, leading to syncopal episodes and possible strokes from embolization to brain
