I think if you can get the paper from UK (letter of need for the speciality) you will be able to get the J1 from UK but if u can't you can as well go back to the UK after your training in the USA provided you have a permanent residence status in the UK, nobody is interested in where you went to, the thing is is that you are out of the USA for 2 years and later can go back. I hope this helps

i think it will be safer to apply from india especially for a j 1 visa because the statement of need will be from india not uk. u dont want to risk refusal of visa fropm uk.

I got my H1 done with no problem in london only 2 questions asked - what r u doing in UK and can i see your degree? thats it . I think risk with j1 is in the visa itself and i doubt if applying in london would add anymore risk(perhaps might even make it less)

aimudia - i am sure u cant do 2yrs anywhere - u will have to do 2 yrs from country of HRR (home residency requirement only)... for stamping dont worry - it doesnt matter where u get it stamped from. ppl go to canada and mexico even. i wud suggest u contact a lawyer for filing - its 1 of a kind expense and worth it.. hope u and bharatphoenix do eventually post where u both match so we can realise the imp of UKCE and MRCP. btw why did u stop ur iview series - it was really good apurva
