IS it safe to assume that an IV drug abuser will always have a DISEASED tricuspid valve?!

There are two type of endocarditis that are caused by two different bugs. Acute is by s. aureus on NORMAL valves and subacute is by viridans s. on congenitally abnormal or diseased valves.
