-A 30 year old woman with a long history of alchohol abuse and barbitutate abuse presents with diffuse colicky pain of the abdomen. Exam reveals numerous surgical scars on her abdomen. She has no photosensitivity, or facial hair. Her urine would probably: A. Contain increased uroporphyrin B. Contain increased coproporphyrin C. Be + for blood with dipstick D. Have excess Urobilinogen E. Turn a portwine color after sitting on a window sill for a short time. Follow up Q's: What disease does this woman have? Why do we never give barbiturates or other P450 inducers to people with this disease? What is the effect of barbiturates on heme synthesis?

Acute intermittent porphyria answer is E urine has increased conc of porphobilinogen and ala synthetase.... BArbiturate increases ala synthesis.
