pap smear shows high grade squamous intraepithelial lesion
which colposcopy and directed biopsies finding indicates for a diagnostic cervical conization.
1. intraepithelial neoplasia
2. negative
3. mild dysplasia
4. carcinoma in situ
5. stage ib invasive carcinoma

The procedure is used for the definite diagnosis of squamous or glandular intraepithelial lesions, for excluding microinvasive carcinomas, and for treatment of CIN. Cervical conization may be performed for further diagnostics following an ASCUS Pap Smear

stage 1b Invasive Ca would be better choice. As Conization is used for Microinvesive Ca. Please correct me if i am wrong.

I think it could have several answers. Because

CIN II, III, and repetitive I and even a carcinoma in situ can be indications for a conization.

So, it could be 1, 3 or 4.

About a 1b invasive CA, it must be treated with radical hysterectomy or radiation.

stage 1b...if I am not mistaken...involoves vagina lower down...so conization not really helpfull, however, in CIS it is not only diagnstic and prognostic...it is theraputic.

indications of cone biopsy

1. disparity between papsmear and colposcopy

2. unsatisfactory colposcopy

3. abnormal endocervial curettage

4. pap smear shows adenocarcinoma in situ

5. microinvasive carcinoma.



so the right answer is carcinoma in situ.
