Q. homeless alcoholic has fever, foul-smelling sputum, poor dental hygiene. XR: cavitary lesion with fluid level in the superior segment of the right lower lobe. sputum grams's stain will find: 1. g+ cocci and variable g+ and g- rods with tapered ends 2. g- fat rods with capsule A&Y?

answer is #1. and bc the alcoholics and especially with poor mouth hygiene are prone to aspirates the content of upper respirotory tract organism and ends up with abcess in their lungs, with multiple organisms including anaerobes

thanks, guest. but isn't the 2nd answer, g- rods with capsules, that is, klebsiella pneumoniae is very typical in the alcoholic?

i would go with first one as in this scenario his poor hygiene is mentioned so aspiration pneumonia. Klebsiella scenario will mention alcoholic in hosp setting with currant jelly sputum
