Which disc is herniated? (Please explain why) 1. Weakness of the quadriceps muscle and an absent knee jerk reflex 2. Pain in the hip and lateral quadriceps, numbness of the anterolateral leg and the webbed space between the great toe, weakness of dorsiflexion of the foot, and normal knee and ankle jerk reflexes (Normal DTR's) 3. Numbness along the lateral and posterior aspect of the calf and plantar aspect of the foot, and an absnet ankle jerk reflex

1. quadriceps weakness/ knee jerk absent-- disk herniated L3-L4 ,, SO root compressed is L4. 2..........? ...? 3. numbness posterior/ lateral aspects , absent ankle jerk reflex-- L5-S1 --- nerve root compreesed is S1

WHAT DRVIRGO IS TRYING TO SAY IS THAT BECASUE THE EFFECTED ROOTS ARE: 1=L4 ROOT 2=L5 ROOT 3=S1 ROOT Although the above roots pass between l4-l5 l5-s1 and s1-s2 ,the disc herniation is like drvirgo mentioned.The explanation is that when a disc herniates ,usually in the posterolateral direction towards the intervertebral foramen,it doesnt effect the nerve root arising from the corresponding level ,because that root has already left at a level above that of the herniation ,instead it effects the one that is about to leave in the next int. foramen. For example,in L4-L5 disc herniation l4 is spared while L5 IS EFFECTED.

Thanks again zaid... great explanation... You are right... the IV discs usually herniate in a posterolateral direction and this is important to know because they end up compressing the nerve root below it.

Good. Posterolaterally the spine is not reinforced by ligaments...that's the reaosn why discs peep out posterolaterally HP
